Trip/Outing Name and Dates:  ___________________________________________________________                 Huntsville Ski Club Coordinator/Leader: __________________________________________________   ASSUMPTION OF RISK AND RELEASE OF LIABILITY   I acknowledge that the Huntsville Ski Club regularly organizes, coordinates and conducts numerous events for its members.  These events include, but are not limited to, a variety of recreational activities, skiing trips, hikes, canoe trips, tennis matches, social events, and other activities for the mutual benefit of the organization and its members therein.   I am aware that during my participation in the above Huntsville Ski Club outing, I may be exposed to a variety of hazards and risks, foreseen or unforeseen, which are inherent in each outing.  These inherent risks include, but are not limited to, the dangers of serious injury and property damage.  I know that such injuries and damages can occur by natural causes or activities, persons, animals, trip members, trip leaders and assistants or third parties.   In consideration of the benefits I receive, I specifically WAIVE, DISCHARGE CLAIMS AND RELEASE FROM LIABILITY any and all claims or demands against the Huntsville Ski Club, its officers, directors, leaders, agents and coordinators, for any injury, damage or pecuniary loss due to my participation in the above event.  I further agree to HOLD HARMLESS the Huntsville Ski Club, its officers, directors, leaders, agents, and coordinators from any claims, damages or injuries by anyone's negligence while a participant on the above identified outing.  I understand and intend that this Assumption of Risk and Release is binding upon my heirs, executors, administrators and assigns, and includes any minors accompanying me on the outing.     *If a minor is participating in the activity, the minor's guardian must sign on the minor's behalf.   Name (please print)                                                              Signature                                                               Date
